Rooted Reach Inc. is a Delaware-based nonprofit organization committed to eliminating barriers that prevent underserved communities from accessing essential resources for wellness and opportunity. We recognize that health extends beyond medical care, encompassing access to nutritious food, stable housing, reliable transportation, and financial security.
Our mission is to reduce Social Drivers of Health disparities through accessible resources, education, strategic partnerships, and community-driven solutions. We work alongside Delaware’s Medicaid and underserved communities, believing that those who experience challenges are also the experts in their own lives.
At Rooted Reach, we don’t just provide services—we build relationships. Every interaction is designed to strengthen both immediate outcomes and long-term self-sufficiency. Through strategic partnerships with local organizations, healthcare systems, and government agencies, we ensure comprehensive support that addresses root causes, not just symptoms.
